A Growing Health System
Southern New Hampshire Health (SNHH) is a strong and growing integrated health care system that serves more than 32 communities in southern New Hampshire and northern Massachusetts. The region’s only clinical affiliate of Mass General Hospital, SNHH is comprised of Southern New Hampshire Medical Center (SNHMC) and Foundation Medical Partners (FMP), a health care model that aligns primary care physicians and advance practice providers with highly-trained subspecialists in one network.

SNHMC, a 188-bed facility with two campuses, offers state-of-the-art resources, including the most advanced technologies for minimally invasive surgery in inpatient and outpatient settings. FMP, one of the region’s largest multispecialty group practices, includes hundreds of providers in 70 community-based practices. Primary care practices include experienced care teams with embedded care coordinators. SNHH also offers the region’s most comprehensive behavioral health services, extending from inpatient and partial hospitalization to outpatient care.
